You are required to submit a signed copy of your prior year federal tax return. If you are a dependent student, we must also have a signed copy of your parents' (who submitted information on the FAFSA)federal tax return in order to complete the verification process. If you have misplaced copies of the federal tax returns, you may request a tax transcript from the IRS. To request a copy of your tax transcript, contact the IRS at 1-800-829-1040.

The tax transcript should be signed by the tax filer and sent to the Office of Student Financial Aid. The tax form 8453 is not acceptable.

2008-2009 Verification Worksheet for Dependent Students
Use this form to complete your Verification requirements if you are a Dependent Student. Print and complete the form. Be sure the form is signed (both the student and parent signatures are required), and submit it to the Office of Student Financial Aid.

2008-2009 Verification Worksheet for Independent Students
Use this form to complete your Verification requirements if you are an Independent Student. Print and complete the form. Be sure to sign the form, and submit it to the Office of Student Financial Aid.

2008-2009 Drug Question Worksheet for Question 31
Use this form if FAFSA Question 31 was left blank, if you answered that you have had any drug convictions, or if you answered that you are not eligible to receive financial aid. Print and complete the form. Be sure to sign the form, and submit it to the Office of Student Financial Aid.

2008-2009 Living Expense Worksheet
Use this form if you were asked to provide your living expenses in your Verification request. Print and complete the form. Be sure that the form is signed, and submit it to the Office of Student Financial Aid.

2008-2009 Nonfiler Certification Form
Use this form to certify that you did not file taxes for the 2007 tax year. Print and complete the form. Be sure the form is signed, and submit it to the Office of Student Financial Aid.

2008-2009 Support Statement
DO NOT Complete this form UNLESS you have been requested to do so. Print and complete the document and submit it to the Office of Student Financial Aid.
