Forms that do not accept electronic signatures can be mailed or hand delivered to the Office of Student Financial Aid and Scholarships.

Loan Forms

  • Loan Cancellation Form
    A student or parent can use this form to cancel all, or a portion, of a loan for Fall 2025, Spring 2026 or Summer 2026. If you need additional funding and previously declined part (or all) of your student loan, please contact our office for assistance.

Verification Forms

  • Statement of Educational Purpose
    Identity Confirmation and Statement of Educational Purpose is to be completed in person at the university or in the presence of a notary.

Miscellaneous Forms

  • Required Course Checklist
    For graduate students taking undergraduate courses and undergraduate students taking graduate coursework. Use this form only if asked to send to advisor or Dean's office for completion.
  • Scholarship Release Form
    Student consent to release information for scholarship eligibility
  • Consortium Agreement Application
    For students attending Appalachian State University and another institution that meets eligibility requirements.
